In our Chorus COVID Protocols Survey, our preliminary results show that 36% of choruses plan to require singers to sign a COVID waiver, 34% do not, and approximately 30% are currently unsure

We simultaneously asked choruses if they would be willing to share the waiver forms they will be using, as well as their names. Thank you so much to the choruses that generously shared their Singer COVID-19 Liability-Waiver and Protocol Forms. These are forms that "hold harmless" the chorus from legal liability, and at the same time detail singer protocols and guidelines for safe rehearsals and performances.   

We've included links to many of those forms below. We curated the submissions to be representative of the sorts of forms most frequently used. We've organized them by chorus type, though many are generic and could be used by any type of chorus. While the forms convey similar information, they vary in length and style so choose an example that works best for your chorus. We also collected a number of broader singer policy agreements but limited our selections here to COVID-only.
